An Ohio municipal tax collection agency has lost a DVD containing tax documents with names, Social Security numbers and birth dates for about 50,000 people.
The Regional Income Tax Agency of Ohio says it discovered that a DVD with 2012 tax documents was missing while preparing to destroy discs in November. The agency posted a notice on its website that the data had been lost Dec. 31.
RITA officials say they've sent letters to affected taxpayers and have offered free identity protection services. An attorney for the agency says there's no evidence that any of the data has been used maliciously.
The attorney says RITA previously lost information on about 700 taxpayers in 2006 when a laptop was stolen from an employee.
The agency serves communities throughout northeast Ohio.
